Market Watch: the UK raised its cost-effectiveness threshold

What changed. NICE's cost-effectiveness range moved from £20,000 to £30,000 per quality adjusted life year, where it had sat for two decades, to £25,000 to £35,000. The change was made by statutory instrument on 2 March 2026 and came into force on 24 March 2026.^{1 2}

The same instrument gives the Secretary of State a statutory power to direct the threshold, which is a structural change as significant as the number itself.

The part most coverage misses. The threshold rise did not happen in isolation. It is written into a bilateral arrangement with the United States published on 2 April 2026, alongside a cut to the VPAG repayment rate and a commitment to raise the net NHS price paid for new medicines by 25 percent from April 2026.³ The same document carries a UK commitment to double spending on new medicines from 0.3 to 0.6 percent of GDP by 2036.

That arrangement is also why UK-origin pharmaceuticals now enter the United States at a zero Section 232 tariff rate rather than the 10 percent originally set.⁴

So a UK reimbursement threshold, a UK price commitment and a US tariff rate are now the same negotiation. That is new, and it is the thing to internalise.

Dates still ahead. Pilot options under the arrangement were to be agreed by 30 June 2026, pilots launch on 1 September 2026, and a replacement pricing scheme is due from 1 January 2029.³

What to do now. Re-run UK health economic models at the new range before submitting. Some indications that were borderline at £30,000 clear at £35,000, and that is a resubmission question, not just a new-submission one.

Then watch the September pilots, because they are where the post-2029 UK pricing system is actually being designed, and the design is being done in public while most companies are still recalculating the threshold.
